MH Connext is a free community mental health service provided by Richmond Wellbeing, for people aged 18+, who have a mental health treatment plan and live in the Perth metro area. This recovery-oriented program connects patients experiencing severe and complex mental health issues to the right mental health care and support at the right time, for up to 24 weeks. Our highly experienced and professional mental health clinicians will provide you with personalised and inclusive support.

By linking in with your GP and using our skills, expertise, empathetic approach, and networks, our team is able to provide individuals with support and guidance to assist them to develop a meaningful life.

Our team draws on various techniques including psychological therapies, therapeutic relationships and compassionate care management; and are able to support people with their psychiatric medication plan.

Our program provides a short-term support service tailored to your needs, that typically involves a review of your personal circumstances, providing emotional and psychological help while connecting you with local support services in a timely way.

MH Connext clinicians will assist you to set goals for your recovery and provide motivation and support you in reaching them.

MH Connext acts as a bridge between the GP and other mental health services. In addition to the care they provide, MH Connext clinicians will assist you to access other services such as:

  • alcohol and other drug services (including harm minimisation)
  • financial counselling and Centrelink support
  • accommodation support
  • online mental health resources
  • community support groups
  • other relevant occupational, disability and culturally appropriate services

In order to be eligible for MH Connext, you must be over 18, and live within the Perth Metropolitan Area.

To access this service, you must be referred by your GP.

GPs in the Perth metropolitan area can refer those experiencing severe and complex mental illness to care management services. Following a referral, an MH Connext Face to Face care practitioner will contact you directly to conduct an initial assessment or will contact your GP to discuss.

This program is free for patients 18 years and over with a Mental Health Treatment Plan.

Perth Central, Perth North/West, Perth North/East, Perth South/West and Perth South/East.

Step 1) Visit your GP to discuss your MH needs, and complete a Mental Health Care Plan (MHCP)

Step 2) Your GP will complete the MH Connext referral form, and send the completed form, along with a copy of your MHCP to intake@rw.org.au

Step 3) Once your referral has been accepted, an MH Connext clinician will contact you to arrange your first meeting, and to assess your situation and how best to support you.
